Live indicator of the cert authority's reachability and health, polled from the /api/health endpoint.

The page polls every 10 seconds. The /api/health endpoint reports services_active, db_ok, schema_version, protocol_version, and a timestamp. Phase 0d adds the real Reviewer wiring; claude_api_key_present flips to true at that slice.
